Your Focus Application Forms For ISSC Contracts

Welcome. On this page, you will find all the different contracts you can apply for, the contract information and the application forms and instructions for documents that need to be completed and uploaded.
We have tried to automate this process as much as possible to cut down on emails and for ease of application.

The Process

Step One: Police vet form

Please note that all new providers need to submit a police vet form even if already approved on a contract with another supplier.

Step Two: Applying for a contract

Step Three: Click on the link for the contract, fill in the form, upload ALL required documents and the application form(s) and submit

NB: Please fill in and send back the police vet form ASAP as we cannot submit the application without this first.

All new providers to Your Focus Ltd need to complete a police check form unless you have a copy of one within the last month of today’s date. If so send us that. If not, please fill in the form and upload in Word format with the attached Identification in the form below.

Step Two: Applying For Contracts

There is a list of contracts below, and under each one is:

  • the eligibility information

  • the application forms

  • the documents you need to submit with the application

Collate all required listed documents for that contract. Fill in the application form(s).

PLEASE only submit once you have ALL the documents attached in the appropriate form. Note. It's recommended that all the necessary documents have been completed and available to be uploaded with forms.

Step Three: Contracts You Are Applying To Be Approved For Under Your Focus Limited

You can apply for more than one contract, but you need to complete the relevant application form for each one.

Click on the appropriate link for the contract you want to apply for, and you will be taken to the appropriate form.

ISSC Sensitive Claims Provider Application

Until recently we would support you along this process every step of the way, and submit the application on your behalf, but ACC have changed the process, and you now need to apply directly to ACC via an online form. It’s not hard and we will guide you.

1.This is process for NEW providers wanting to become ACC registered for Sensitive Claims

Step One: RealMe

You will need a RealMe account and here is the link to sign up for a RealMe log in (if you don’t have one already). This is a secure gateway used by government agencies that secure your privacy and verify your identity.

Step Two: Gather all of your required documentation and save them into a folder as you will need to upload these into the online application form.

You will be referred to as a New Named Provider

Required Documents:

• Academic record of your professional training

• Your CV which shows:

o Your experience working with individuals who have experienced sexual abuse or assault.

o Competency and skills gained in the services you are applying for.

NB: Not required for Clinical Psychologists and Psychiatrists

• Annual Practicing Certificate and / or Professional Body Membership evidence

o This can either be a certificate copy uploaded in this application or you may provide a link.

• Supervisor's details (Name and ACC provider number) and their Annual Practicing Certificate

• Supervisor's written agreement for those applying to be a provisional Named Service Provider

Step Three: Online Application Process

It is quite easy and straightforward, and you can stop it at any time and return to it at a later time, but I would advise you to have everything prepared and just do the application in one go.

If there is no option to upload a document, upload under ‘other’. If a field does not apply to you (like the academic record or the supervisor’s contract, put in N/A.



Documents you will need to upload when applying online:

  • Your Professional Body membership (this is not the annual practising certificate, but your actual membership certificate)

  • Your current annual practising certificate (this is your yearly annual practising certificate)

  • Your CV (outline any trauma training and experience) if not already sent to us.

  • Copies of your qualifications (certificates)

  • Academic transcripts

  • Copies of change of name if applicable

  • Copies of residency if applicable

  • If you work with clients under 18 years old, you will need a Child Safety Check.

Step Four:

The form, when submitted, goes to ACC who will either approve it or ask for additional information or decline it.

Once approved, you can send us the confirmation of registration as your designated supplier, and we will start our onboarding process with you.

This includes mentoring on the process, training videos for report writing access to the provider log in page and all of our resources etc. To speed up your access to accepting referrals, we will be sending you our Police Vet form. You fill it in and send this back asap so that we have the results and can onboard you as soon as you’re accepted. Otherwise, there can be further delay.

Provisional Providers Note

The applicant must have at least the equivalent of 12 months full-time post graduate experience working in mental health (not including clinical placements and internships).

The applicant needs to have a written agreement/contract between themselves and the appropriate ACC registered supervisor, which they must submit with the application.

  • Your Professional body membership certificate (not the APC)

  • Your CV

  • If you work with clients under 18 years old, you will need a Child Safety Check.

ISSC Application Form To Deliver Supported Assessments

Forms below include:

  1. A psychologist or psychiatrist applying to do ISSC supported assessments, not already approved

  2. A psychologist or psychiatrist applying to do ISSC supported assessments. Already approved for this contract and wanting approval under Your Focus Ltd

  3. A psychotherapist or counsellor applying to do ISSC supported assessments, not already approved as an assessor

  4. A psychotherapist or counsellor applying to do ISSC supported assessments, already approved and wanting to come under the Your Focus contract

Contract Information

Unless you are a psychologist or a clinical psychologist, you will likely need to be under a provisional registration process to do supported assessments.

This means you need to.

  • Complete the application form and attach your CV

  • Have an ACC supervisor who holds the contract and will supervise you doing supported assessments. (we have some contacts for this if you need them)
