All new providers to Your Focus Ltd need to complete a police check form unless you have a copy of one within the last 3 months of today’s date. If so, send us that. If not, please fill in the police check form and upload with the attached Identification required in the form below. Please note that all new providers need to submit a police vet form even if already approved on a contract with another supplier.

If you work with clients under 18 years of age, you will need a Child Safety Check.
